الضعف الجنسي

​

العجز الجنسي هو عدم القدرة على الانتصاب. أو عدم القدرة على الاحتفاظ بالانتصاب لفترة كافية للحصول علي ممارسة الجنس الطبيعي .

أما الضعف الجنسي فيقصد به عادة قلةعدد المرات التي يستطيع فيها الشخص أداء جماع كامل وطول الفترة الزمنية بين كل جماع وآخر

بمدة قد تصل إلى عدة .أشهر. وقد يطلق على الضعف الجنسي اوالبرود الجنسي هو عدم استجابة الشخص ـ سواء الزوج أو الزوجة ـ عند الجماع بدرجة كافية للإثارة الجنسية

وقلة الرغبة في أداء العملية الجنسية أوالنفور منها كلية وجميع ماسبق يختلف عن العقم الذي يعرف بأنه عدم القدرة على الإنجاب بعد مرور عام على إتمام لقاءات جنسية منتظمة 

والقذف السريع حالة منتشرة في كل انحاء العالم بنسبة قد تصل الى حوالي 40% من الرجال وهي الاكثر شيوعاً بين كل الامراض الجنسية لديهم

مازالت الآراء حول تحديدها غير موحدة ومثاراً للجدال والنقاش في الاوساط الطبية

فبينما يعتقد بعض الاطباء ان كل قذف يأتي خلال اربع او خمس دقائق من ابتداء الجماع يشكل قذفاً سريعاً يرفض البعض الآخر ذلك

ويدعون ان تحديد القذف السريع يجب ان يقاس من وقت الولوج في المهبل الى وقت القذف ويكون عادة اقل من دقيفة في 90% واقل من 30ثانية في 80% منها من الحالات

كما اظهرت دراسة حديثة اجريت على 110رجال يشكون من القذف السريع في هولندا ولكن بعض الاخصائيين يرفضون هذا التعريف ويضيفون في تحديده

بعداً عملياً يخصون فيه المرأة بالدور الاهم، فلا اهمية للقذف السريع بالنسبة لهم الا بقدر ما يحرم المرأة من اللذة بالاستمتاع بالجماع ونشوة الوصول الى القمة في اكثر من 50% من العلاقات الجنسية.

ولكن هذا التحديد ليس منطقياً لان المسألة نسبية وتتوقف ايضاً على سرعة او تباطؤ التجاوب عند الزوجة، فان التحديد النهائي يبقى صعباً لاسيما اذا اخذنا في الاعتبار ان نحو 27% من النساء قد يصلن الى قمة النشوة أو الإيغاف في مدة لا تزيد على دقيقة واحدة وغيرها تحتاج الي اكثر من 5الي 15دقيقة من التهيج الجنسي للحصول على لذة الجماع،

وان نجاح المجامعة واستمتاع كلا الزوجين بها قد تحصل في اقل منه 50% من الحالات وتعتبر العلاقة الجنسية ناجحة بالنسبة إليهم.

وامام هذا الجدال العنيف حول تحديد سرعة القذف اعتبرت الجمعية الامريكية للامراض النفسية ان اي قذف يحصل بدون اي لذة او تدنيها قبل او اثناء ومباشرة بعد الولوج بدون ان يرغب المريض في حصوله او السيطرة عليه يجب ان يعتبر قذفاً سريعاً او مبكراً.

وفي رأيي الشخصي وحيثما تختلف المدة ما بين الانتصاب او الولوج والقذف في مختلف انحاءالعالم وما قد يرضي الزوجين الاوروبيين او الامريكيين لا يرضي الزوجين العرب مثلاً في هذا المجال فلنفسح للزوجين انفسهم في تحديد سرعة القذف وتأثيره على علاقتهما الجنسية والمضايقة منه والحاجة الى معالجته،

:حسب توقعات منطقية لديهم.

المرأة والقذف السريع

للقذف السريع تأثير سيىء في نفسية الزوجة اذ انه يسد امامها سبل اللذة بعد اثارتها، ويحرمها من اطفاء نار رغبتها بعد اشعالها فيتولد لديا شعور المرارة

اسباب القذف السريع

منذ اكثر من مائة سنة يحاول الاخصائيون والعلماء توضيح اسباب القذف السريع وقد كان الاعتقاد السائد في اوائل الاربعينات ان سببه الرئيسي عصاب متعلق بصراعات نفسية باطنية وافضل علاج له التحليل النفسي.

وفي سنة 1943م دحض الدكتور شابيرو هذه النظرية واقترح بدلها اعتقاده الراسخ ان سبب القذف السريع نفسي وبدني يرتكز على ارتباك فكري وخلل في آلية القذف. وقام بعض الاخصائيين في المجاري البولية والتناسلية بانتقاد هذه النظرية مدعين ان العوامل الاساسية لحدوث هذه الحالة هي ارتفاع حساسية الحشفة وقصر لُجيم القضيب وتشوهات في الاحليل وان افضل علاج لها هي استعمال مخدر على الحشفة وقطع اللجيم وتدخير الاوكيمة المنوية داخل الاحليل. ولكنه للاسف لم تنجح هذه الوسائل واستمر البحث عن الاسباب المجهولة.

وبين 1950و1990م تحولت الاراء الى اعتبار القذف السريع سلوكا مكتسبا ومدروسا ففي 1970م احدث الطبيبان الامريكيان "ماسترز" و"جونسون" ضجة اعلامية عالمية عند نشرهما نتائج ابحاثهما عن العجز الجنسي وسرعة القذف وشددا على اهمية القلق عند المجامعة والخوف من الفشل وتأثير التطبع الجنسي.. فكان اعتقادهما ان الخبرات الجنسية الباكرة تلعب دوراً كبيراً في رسم نمط الانفعال الجنسي مستقبلا فالممارسة الجنسية السريعة مع المومسات او البنات تؤدي الى اكتساب انعكاسات عصبية تؤدي الى سرعة فائقة وحساسية شديدة تجعلان القذف قبل الاوان امراً مألوفاً ونمطا متواصلا. ومن الاسباب الاخرى المقترحة الخوف

من الحمل والاكتئاب واليأس وكره الزوجة والرغبة الباطنية لمعاقبتها وغيرها.

طرق فاشلة

​

وعمت في هذه الفترة وسائل عديدة للتحكم بالقذف منها "التوقف عن مزاولة التهيج الجنسي عدة مرات قبل القذف، وطريقة الضغط المتقطع على الحشفة لمنع القذف مؤقتا ومتابعة المجامعة والضغط مجدداً على الحشفة كلما شعر الرجل بقدوم القذف. وكانت النتائج الاولية لهذه الوسائل العلاجية جيدة واتبعت في كل انحاء العالم ولكنه للاسف انخفض نجاحها مع مرور الزمن وفشلت اغلبها في غضون 3سنوات وعممت عندئذ طرق جديدة للعلاج منها العلاج النفسي والتطبيقي وغيرها ولكنها كانت فاشلة.

وفي اوائل التسعينات جرى تحول مفاجىء وهام في تفهم آلية سرعة القذف بناء على دراسات عالمية تمت على الحيوانات وعلى اشخاص مصابين بهذه الحالة

واتجه التفكير الطبي الى الدماغ والمواد الكيماوية داخله وتبين ان بعض هذه المواد مثل مادة السيروتونين الموجودة في بعض مناطق الدماغ المسؤولة عن القذف لها دور كبير في كبحه وتأخير حدوثه بينما مادة اخرى هي "اللاوكسيثوسين تحثه وتدفعه الى الحدوث السريع. وتبين ايضاً ان التغيرات في تركيز تلك المواد الكيماوية في الدماغ قد يكون وراثي المنشأ وتثبتت هذه النظرية العصبية الحيوية بالنجاح الباهر التي حققه استعمال بعض العقاقير التي تزيد تركيز

مادة "السيروتونين" في الدماغ في معالجة سرعة القذف، كما سنشرحه لاحقاً.

افضل علاج لسرعة القذف
