ما هو العقم وما هي انواعه؟

يعرف العقم بأنه إنعدام في القدرة الإنجابية بشكل مطلق، وقد أخذ العقم بحالاته المستعصية أو مستحيلة العلاج في التلاشي حيث أصبحت العديد من الحالات الآن يمكن علاجها بفضل التطور الطبي الحديث، ومن أمثلة ذلك الإنسداد في الأنابيب عند المرأة، والندرة في النطف المنوي لدى الرجل.

وأنواع العقم تتلخص في عقم أولي وعقم ثانوي وعقم الرجال، والعقم الأولي يخص المرأة حيث يصيبها عند بداية الحياة الزوجية والجنسية، والعقم الثانوي هو الذي يحدث بعد الإنجاب الأول أو بسبب الإجهاض أو الحمل الذي يحدث خارج الرحم، أما عقم الرجال قد يحدث بسبب ضعف الحيوانات المنوية وعد قدرتها على تخصيب البويضة.

 

أسباب العقم

بالنظر إلى الرجل نجد أن الأسباب التي تعمل على إعاقته سواء أكان ذلك بشكل مؤقت أو بشكل دائم، تعود إلى ثلاثة عناصر:

عنصر هرموني: ويتمثل في خلل في وظائف بعض الاجهزة كالغدة النخامية أو الغدة الدرقية والبنكرياس وغيرها.

 عنصر الانسداد: ويكون ذلك عن طريق الانسداد في الحبل المنوي كعيب خلقي، او وجود التهابات في جهاز البول التناسلي وغيرها

 عنصر الخصية: قد يكون السبب هو عدم وجود الخلايا التي تنتج الحيوان المنوي، أو وجود تليفات بالخصية والتي قد تحدث عند مرحلة البلوغ كنتيجة لالتهاب الغدة النكافية، أو إهمال الخصية المعلقة وعلاجها وغيرها.

أما بالنظر إلى الأنثى نجد أن العقم يختلف لديها من حيث النوع فالعقم الأولي يكون نتيجة إلى مرض هرموني أو بسبب توقف نمو الأعضاء التناسلية نتيجة لعيب خلقي، وذلك بالإختلاف عن العقم الثانوني الذي يكون نتيجة إلى عملية إجهاض أو إلتهابات أصابت رحم المرأة أو قناة فالوب أو نتيجة إلى مضاعفات أثناء الولادة الأولى وقد يكون العقم نتيجة إلى:

أمراض المبيضين وتكون نتيجة خلل في الأداء الوظيفي للمبيض وقد تكون بسبب التقدم في العمر.

الأمراض التي تخص الرحم: وهي عادة ما تكون تشوهات خلقية أو إلتصاقات حدثت بالرحم نتيجة إلى وجود إلتهابات به.

إنسداد قناة فالوب وهي عادة ما تكون إلتصاقات بسبب عملية جراحية أو وجود إلتهابات في الحوض.

أسباب غير معلومة: يعني ذلك أن يمر عامين على العلاقة الزوجية المنتظمة بدون حمل ومع ذلك لا تكون أسبابمعروفة أو عضوية وقد فسرت بعض الدراسات إحتمالات هذه الأسباب كالآتي

في عنق الرحم وهي كريات دم بيضاء  WBC  وجود*

عوامل الوراثة الجينية*

عناصر مناعية في بطانة الرحم*

نقص الحديد والفيتامينات*

العوامل النفسية: وهي تختلف من الرجل والأنثى وتختلف من زوج إلى آخر ولكن في الأغلب يكون السبب في التوتر

الفحوصات

إن إجراء تحليل السائل المنوي للرجل قد يقصر الطريق بنسبة كبيرة حيث يتم تخطي 40% من أسباب العقم، وبالإنتقال إلى المرأة نكون أكملنا شوطاً كبيراً وإذا أظهرت الفحوصات سلامة المرأة فننتقل إلى أفكار علاجية أخرى.
